The guard `not (k <= 1 or k >= ns1)` compared an integer k to a binary ns1 (:binary.encode_unsigned(N - 1)). So `k >= ns1` was always false. Since HMAC-DRBG output is truncated to N's bit length, it wasn't an issue in practice.
